Problem:
Operators and maintenance personnel can enter the defined hazard area of the ACCUSTAK® through the stack exit area without locking out the machine as directed in the operator manual. Entering the hazard area while the machine is energized exposes personnel to a risk of injury.
When the Hazardous Energy Lockout procedure in your operator/maintenance manual is followed, the ACCUSTAK is de-energized for safe entry.
If these bolts
have loosened up, we recommend that you retighten them at the torque
value recommended below.

action:
Alliance offers an optional stack exit light curtain retrofit for the ACCUSTAK. The optional safety light curtain is recommended for installations that require additional safeguards beyond required lockout/tagout procedures.
This safety light curtain allows stacks to exit but causes the safety circuit to de-energize the ACCUSTAK if operators and/or other personnel enter the hazard area through the stack exit. The light curtain will not affect entry into the hazard area from other areas of the machine.
