Description of Issue
Lift motion
(upstream, downstream and side-to-side) is tracked with two guide
assemblies that are mounted to the sides of the lift frame
assembly. The guide assemblies keep the lift in its proper location
while it travels up and down inside the Jumbo ACCUSTAK main frame.
They are also used as connection points for the chains that raise
and lower the lift. Over time, the guide assemblies can become
fatigued. Failure of the guide assemblies on both sides could cause
the lift assembly to freefall to the floor.

Procedure for Correction
Lift guide assemblies on both sides
of the machine should be inspected for cracks and fatigue using the
following procedure and replaced immediately if any damage is found.
An improved design is available for retrofit and is recommended even
if no damage is found.

Raise the lift and insert the mechanical lockout pins.
Complete the lockout following the procedure in the operator manual
supplied with the machine.

Refer to the following photograph for the location of the
lift guides. Each machine has two; one is on the operator side and
one on the drive side. The easiest way to find the lift guide is to
follow the lift chains that wrap around the the lift cylinder idler
sprocket until they connect to the guide assemblies bolted to the
side of the lift.

Inspect the lift guide and chain attachment for signs of wear
or fatigue. If any are found, call Alliance Customer Service
immediately.
Inspect the lift guide assemblies
and chain attachments once a month as part of your periodic
maintenance.
